In the assessment of patients having prolonged hospital stays from incorrect charting information and lack of care we will address any changes that might be needed. In changing the process as a manager, we would consider a three-step theory presented by Kurt Lewin simply unfreezing, change, refreezing[Cum15].
In reviewing protocols and current practice, we would determine the best practice in organizing a unified approach to staff collecting intake and output data and establish a process that will benefit patient care.
In order to unfreeze current staff behavior and define the issue at hand by analyzing current workloads we have determined that night shift will take weights of the patients between two and six am to maintain established times daily and to obtain a dry weight if possible “change”.
And intakes and output to be charted at five am and five pm to produce a uniformed practice, refreeze. Leadership will work with nursing staff and facilitate the changes need to support patient outcomes.2.Strategies and Rationalea.The results found in assessing patient care or lack of is concerning and the reason for the need of immediate change in practice.

Expected Outcomea.After discussed with leadership and upper management we will first do a baseline audit of electronic medical record (EMR) and the bedside flowsheet to determine the previous consistency rate of intake, output and weights charted.
Once that has been concluded we will move forward in the change process of nursing procedureand protocols. It is expected that there will be some resistance and confusion at the beginning and some charting discrepancies but as we continue to reeducate and reinforce behavior we should have a successful changeover.
